Digest Semantics

What the hash proves

document_digest SHA-256 over the canonical MDX payload, with its own digest line zeroed before hashing.
registry_root SHA-256 over sorted proof metadata: slug, source path, title, version, state, date, scope, and digest.
claim_boundary Hashes prove identity and review traceability. They do not independently prove a technical claim true.
